چکیده

• Vol 7 • October 2005 Normal-Tension Glaucoma Open-angle glaucoma with normal intraocular pressure, also known as normal-tension glaucoma, is estimated to constitute 10–25% of all glaucoma cases. NTG is a diagnostic challenge, as various lesions affecting the visual pathways including compression by normal or abnormal intracranial blood vessels can mimic its clinical findings. In recent years, several studies have shown a statistically significant percentage of patients with NTG who were shown to have compression of the optic nerves by normal or abnormal carotid arteries compared with controls or patients with high pressure glaucoma. We report two patients who initially presented with clinical signs imitating bilateral NTG, with non-progressive binasal visual field defects due to optic chiasm compression by low positioned anterior cerebral arteries. We show the radiologic findings and discuss possible explanations for these observations.

Neurological Hemifield Test in Binasal Defects.

We read with great interest the paper by McCoy et al. validating the neurological hemifield test (NHT) to detect and classify visual field loss caused by chiasmal or postchiasmal lesions. In this paper, more than 60% of glaucoma eyes misclassified as neurological according to the NHT score criterion had binasal defects. Therefore, interpreting binasal hemianopia is a particular challenge. To em...

An Unruptured Anterior Communicating Artery Aneurysm Presenting with Left Homonymous Hemianopsia: A Case Report

Unruptured cerebral aneurysms sometimes present with visual symptomsdue to compression of the visual pathways. However, until now, unruptured anterior communicating artery (ACoA) aneurysms presenting visual field defects have been extremely rare. The authors report the case of a 51-year-old woman who presented with left homonymous hemianopsia. Radiological findings demonstrated an ACoA aneurysm...

Microvascular Decompression of the Optic Nerve for Paroxysmal Phosphenes and Visual Field Deficit.

BACKGROUND Microvascular decompression surgery is standard neurosurgical practice for treating trigeminal neuralgia and hemifacial spasm. Most other cranial nerves have been decompressed for paroxysmal intermittent hyperactivity of the affected cranial nerve or in very long-standing compressions to treat cranial nerve hypofunctioning. CASE DESCRIPTION We describe a case of intermittent paroxy...

Radiologic-clinical correlation. Junctional visual field loss.

Nerve fibers originating in the retina follow a specific topographical arrangement in the optic nerve and chiasm. Compressive lesions at the junction of the intracranial optic nerve and optic chiasm can produce characteristic visual field defects. Optic nerve involvement at the junction of the optic chiasm results in unilateral visual field loss. If fibers from the inferonasal retina of the con...
